At Yahweh Care Day Centre, our Junior Explorers program is lovingly designed for children aged 12 to 18. What to Bring

  • Packed lunch and water bottle (labelled) If you're unable to bring lunch, we offer a healthy lunch and afternoon tea for $25. Please speak to our staff to arrange this option.

The NDIS (National Disability Insurance Scheme) helps fund supports and services for people with disability. At Yahweh Care, we work with you to match our programs to your NDIS goals across Core Supports and Capacity Building budgets. We also assist with service agreements, reporting, and plan reviews.

Please bring a packed lunch, water bottle (labelled), any required medications with instructions, a change of clothes (if needed), and favourite sensory or comfort items. If you're unable to bring lunch, we offer healthy meals and afternoon tea for $25 per day.

We offer tools like daily journals, goal-tracking updates, and personalised participant profiles to help families stay informed and involved in every step of the care journey.

Access essential forms such as the enrolment and intake form, media/transport/medical consent, emergency contacts, NDIS plan upload (optional), and a feedback and complaints form — all in easy-read formats.
